This agreement is made as of the date entered below ("Today's Date") between individual signing below with their Full Name and Birthdate, (hereinafter known as "Client") and FJS Solutions, LLC of 21213B Hawthorne Blvd 3029 Hawthorne, CA 90503 (hereinafter known as “Coach”).

REwrite sessions will be conducted by Flora Sinha. This contract is between FJS Solutions, LLC and Client.

Total: $4,500 USD if paying in full
If doing a monthly payment: Six payments of $850 (total of $5,100 USD) or Eight payments of $650 (total of $5,200 USD). 

*Any approved promotional pricing, discounts, or special offers applied at checkout are considered part of this Agreement.

Both parties agree to the following:

Coaching is a collaborative process with an ongoing relationship as Client and Coach. This coaching experience supports the Client in establishing new behaviors. The coaching agenda is based on an original format formed by the Coach, FJS Solutions, LLC. The role of the Coach is to help the Client progress toward achieving a goal that will be stated at the beginning of the session. 
 

The Client and Coach agree to engage fully in each coaching session.

The Client understands that coaching does not serve as therapy, counseling, consulting, or medical evaluation/advice. If you have medical concerns, please contact your primary care provider as soon as possible. If yo are experiencing a medical emergency at any time, please call 911 for EMS services.

The Client understands that REwritten is a 6-month private coaching experience. It is a MD-Led Identity & Physiology Reconstruction. This is not considered medical treatment. REwritten is structured guidance designed to help you stabilize your physiology through education, behavior recalibration, and systems change.

CONFIDENTIALITY

The Coach agrees to keep all conversations and information with the Client private and confidential. No personal information will be shared with anyone unless the Client grants permission. Coach may share stories that do not divulge personal information in any way for educational purposes only. Exceptions to this agreement will be made if there is any imminent threat or serious injury to oneself or someone else.

The Client agrees to keep the intellectual property and learning material adaptations during coaching sessions with FJS Solutions, LLC confidential and will not use the material directly for personal financial gain. Not abiding by this will result in legal action.

COACHING COMMITMENT

The Client and Coach acknowledge that the Client desires to make a behavioral change or a type of improvement in her life. There is an understanding that behavioral change takes time to implement and sustain. The pace of change varies among individuals. As such, the Client and Coach agree to 14 1:1 50 minute sessions + Voxer support from 8-5pm PT Monday-Friday (messages outside of this window will be addressed the next business day) with the expectation that learned behaviors will be practiced after the coaching series ends.

COACHING SESSION PROCEDURES 

REwrite sessions will occur virtually via Zoom video conference.

The Coach and Client agree to adhere to established appointment times.

If the Client is more than 15 minutes late to an appointment without prior notice of at least 24 hours prior to the scheduled session, the Coach will assume that the appointment is canceled. One reschedule in this scenario will be allowed within the contracted time period. Incidents thereafter will result in the Client being billed the full amount and forfeited the session.

If the Coach is more than 15 minutes late to an appointment without prior notice of at least 24 hours prior to the scheduled session, the Client may assume that the session is canceled.  The Coach is obligated to reschedule and Client will not forfeit missed session.

The Client and Coach agree to reschedule an appointment at least 24 hours in advance. Notifications will be sent via email address or Voxer number provided in the intake form.

COACHING FEES

Specific coaching fees and packages are outlined as follows:

-REwritten's 1:1 Coaching with Dr. Flora includes fourteen 50 minute 1:1 REwrite sessions over six months.

-Fees must be paid in payments as chosen at clarity call. First or full payment is due upon signature of this contract after the free 30-minute Let's Talk It Out Call. An absence or delay in payment may result in a forfeited session. If doing monthly payments, they must be made via auto-pay to FJS Solutions, LLC. 

REFUND POLICY
Choosing a coach is an important decision and it’s important to find a good fit. This is the reason for the free 30-minute Let's Talk It Out Call prior to starting the program. A full refund is available if the Client decides to cancel their commitment after the first fifty minute session. To receive this refund, the Client must email FloraSinhaMD@gmail.com in writing within 24 hours after the FIRST session. Failure to do so will forfeit the Client's opportunity for a refund and the Client will be billed fully.
